What basic and comprehensive packages usually include
| Package level | Typical contents | Typical list price (editorial estimate) |
|---|---|---|
| Basic | Height, weight and BMI; blood pressure; blood tests (glucose, cholesterol, kidney and liver function); urine test; sometimes vision test | RM130 – RM600 |
| Standard / mid-tier | Basic tests plus chest X-ray, resting ECG, doctor consultation, sometimes tumour markers or abdominal ultrasound | RM500 – RM1,100 |
| Comprehensive / executive | Mid-tier plus treadmill stress test, ultrasound or CT/low-dose imaging, cancer screening (Pap smear, mammogram, PSA), specialist review | RM1,000 – RM2,200 and above |
Ranges are editorial estimates from hospital and clinic package pages seen in 2026; many are promotional prices (for example a mid-tier package promoted at RM500 against a listed RM1,080). Contents vary widely under the same name, so compare the test list rather than the label.
Free screening: PERKESO Health Screening Programme
- PERKESO (SOCSO) offers free health screening to eligible contributors to identify high-risk workers for non-communicable diseases such as heart disease, diabetes and cancer (cervical and breast).
- Published criteria: Malaysian citizen, actively contributing to PERKESO under the Employment Injury Scheme, aged 40 and above; some programme pages also mention contributors aged 30 to 39 with health risks.
- You check eligibility and book a slot at a registered panel clinic through the SEHATi app (Google Play and App Store).
- The programme has run in phases (relaunched 1 June 2022; HSP 3.0 ran 16 May to 31 December 2023), so confirm current eligibility and availability in SEHATi before relying on it.
Other free or subsidised options include government health clinics and Ministry of Health screening campaigns, and some employers or insurers fund annual check-ups.
Tax relief for medical check-ups
- Self, spouse or child (YA2025): up to RM1,000 for a complete medical examination, disease detection tests, self-testing devices and mental health examinations or consultations, combined in one cap. Disease detection tests were added from YA2025. This sits inside the overall RM10,000 medical relief group.
- Vaccination and dental: a separate RM1,000 each within the same RM10,000 group (cosmetic dental work is excluded).
- Parents or grandparents: a complete medical examination including vaccination counts up to RM1,000 within the RM8,000 relief for parents’ medical expenses.
- LHDN’s explanatory notes require the examination to be carried out at a hospital or by a registered medical practitioner, with an official receipt, incurred in Malaysia.
These are the YA2025 (filed in 2026) figures. We did not find a published change for YA2026; check the current LHDN tax relief page and Public Ruling before filing, or ask a tax agent.
How often should you screen?
- General guidance from Malaysian hospitals is a regular check-up at least once a year, with the content tailored to age, sex, family history and existing conditions.
- Cancer screening (Pap smear, mammogram, bowel, prostate) follows separate age-based schedules; ask your doctor which apply to you.
- Eye screening is often suggested for people over 50 and for those with high myopia, diabetes or a family history of glaucoma.
- If you have symptoms, see a doctor rather than waiting for the next scheduled screening.
How to compare packages
- Compare the individual tests, not the package name.
- Check that a doctor consultation to explain results is included, and whether the report is in a form you can share with your regular doctor.
- Ask what happens after an abnormal result, and whether follow-up tests are billed separately.
- Check whether your employer, insurer or PERKESO covers part of the cost, and keep the official receipt for tax relief.
- Screening finds possible risks; it does not guarantee that you are free from disease.
